We need some information to grant you a MyAltaMed account. After you submit this request, it may take up to a week for processing and verification. When you're approved, you will receive an email or a letter with your activation code and instructions on how to activate your MyAltaMed account. Children (less than 12 years old): To request access to view a child’s chart, you must have your own MyAltaMed account (sign up below). Log into your account and fill out this form: "Minor Record Access Request." Access will end when patient turns 12 years old.
Adolescents (12-17 years old): To protect the privacy of adolescents AltaMed currently does NOT provide access via MyAltaMed. To request medical records of patients ages 12-17, please send an email to: Recordrequest@altamed.org OR, fill out a paper authorization form: click here.
